Individual Clients:
Duly completed Calyx account opening form
2 Recent passport photograghs
Copy of current and valid International Passport, Drivers License, National ID or any other form of identification.
Duly completed CSCS Account opeing form
Copy of recent utility bill issued within the last 3 months, of either NITEL, PHCN e.t.c

Corporate & Institutional Clients:
Duly Completed Calyx account opening forms 
Certificate of Incorporation
Board resolution to open an investment account and introducing the signatories to Calyx Secuirites
CSCS Account Opening form
Transfer Forms (Minimum 10 copies)
Mode of Identification of the account signatories
Two recent passport photographs of the signatories
